Background

Changes in the task and funding responsibilities from the health regions in 2006. A report from the regional work group from Helse Midt-Norge 31.5.06.

Purpose and objectives

To create best possible patient pathway where the total amount of resources are well applied. To develop and improve a cooperation model that focuses on interaction between the hospital and private rehabilitation institutions.

Methodology and actions

Baseline survey. Documentation of and implementation of new routines. Audit of all rehabilitation institutions. Establish good supervision and guidence services. Development of standards. IT systems implemented.

Results and conclusions

The pilot is externally evaluated. And is finalised with a report at the end of 2008.
